In its most severe form, serotonin syndrome can resemble neuroleptic malignant syndrome, which includes a high fever, muscle rigidity, autonomic instability, and possible fluctuations in vital signs and mental status.
• Watch patient closely (especially children, adolescents, and young adults), for suicidal tendencies, particularly when therapy starts and dosage changes, because depression may worsen temporarily during these times and lead to suicidal ideation.
• Monitor patient for bleeding, especially if patient also takes aspirin, an NSAID, or an anticoagulant. Bleeding can range from ecchymoses, hemtomas, epitaxis, and petechiae to life-threatening hemorrhage.
• Discontinue fluvoxamine therapy gradually, as ordered, to prevent unpleasant adverse reactions.
